Patches was killed, Onion Bro was stuck in jail. With no one to save him, he lived the thug life, aimed high, died in sewers. To save Greirat from Irithyll, you have to save Siegward from the well outside the Church before the Cathedral of the Deep by buying his armor from Patches and giving it to him. Greirat dies regardless, but after giving his ashes to the Handmaid, she will have everything he sells, including the new loot he never returned with.
